Concerned by critical backlogs in appeals court cases coming from most of New York City and its suburbs, Gov. Mario M. Cuomo and legislative leaders are seeking to expand New York State's appellate court system. Legislative aides say leaders of the Democratic-controlled Assembly and Republican-controlled State Senate will introduce, possibly this week, proposed amendments to the State Constitution that would authorize the first major change in the appellate courts in a century. The change would add a fifth department to the four that now form the Appellate Division, the state's intermediate appeals court and the last stop for most cases.

For Dr. Michael Elia, a Westchester orthopedic surgeon, the national debate over health care has come to this: he has taken the M.D. license plates off his sports car. "People see you driving around in a little convertible with M.D. plates and they get hostile," said Dr. Elia, who practices at Lawrence Hospital in Bronxville. "It's just a $14,000 Miata, but people see the plates and assume its a $60,000 Porsche. There's such resentment against doctors; why make it worse?"
